Application for Legal Assistance

LSNF offers service delivery in a variety of ways.  In addition to direct representation, we operate private attorney involvement and telephone advice programs, and we conduct legal clinics.

In order to be determined eligible for these services you will need to complete and submit the necessary application forms.   Feel free to download the following forms and mail to the appropriate LSNF office.  Click on the appropriate link to see which LSNF branch serves your area and the address to mail your application:  TallahasseeFort Walton Beach, Panama City, Pensacola, or Quincy

  • Standard Client Intake Application (Word Perfect format) (PDF format) Required Make certain to provide all information, especially your social security number, date of birth, and all income.  You must sign your name in the space provided, attesting to your US citizenship.

Once your application reaches us it will be reviewed to determine whether or not we can handle your case.  Please direct all inquiries to the appropriate office.
