Application for Legal Assistance

LSNF offers service delivery in a variety of ways.  In addition to direct representation, we operate private attorney involvement and telephone advice programs, and we conduct legal clinics.

In order to be determined eligible for these services you will need to complete and submit the necessary application forms.   Feel free to download the following forms and mail to the appropriate LSNF office.  Click on the appropriate link to see which LSNF branch serves your area and the address to mail your application:  Tallahassee,  Fort Walton Beach, Panama City, Pensacola, or Quincy.

STEP ONE:

• Standard Client Intake Application (Word Perfect format) (PDF format) Required Make certain to provide all information, especially your social security number, date of birth, and all income.  You must sign your name in the space provided, attesting to your US citizenship. 

STEP TWO:

• Intake Fact Sheet (Word Perfect format) (PDF format) Required Tell us what you think your legal problem is AND what you want us to do.

STEP THREE:

• Public Assistance Survey (PDF format) Required Do your best filling out this survey and return along with the documents above.

STEP  FOUR:

• Alien Form - IF NOT A U.S. CITIZEN, but a legal resident  (Word Perfect format) (PDF format) Required

STEP FIVE:

• Divorce Application - if services involve a divorce  (Word Perfect format) (PDF format) Required     OR

• Bankruptcy Application - if services involve a bankruptcy (Word Perfect format) (PDF format) Required

STEP SIX:

• Mail or Fax your application forms to the Legal Service office nearest you (click on the city closest to your residence for address and fax number information) - Fort Walton Beach, Panama City, Pensacola, Quincy, and Tallahassee.

Once your application reaches us it will be reviewed to determine whether or not we can handle your case.  Please direct all inquiries to the appropriate office.
